How to Register

If your permanent home address is within our practice boundary, you can now register online.

Alternatively you can collect paper registration forms from our reception.

Patients will be allocated a Named GP, however you are free to book an appointment with any of our GPs. Please bring a form of photographic ID with you when you register. This is to help us confirm we are accessing the correct records.

Temporary Residents

If you are temporarily living in the area, (for up to 3 months), and need medical treatment you can apply to register as a Temporary Resident, enabling you to take full advantage of all our medical services. Please fill out the Temporary Registration form and return to the surgery.

Change of Personal Details

If you change your name, address or telephone number, please give full details to reception, including your postcode.  Please remember to tell us if details for other members of your family have also changed.  If you move outside the practice area you will be asked to register with a doctor nearer to your new home.

We are in the final stages of having our website completely redesigned to meet the most recent NHS and Accessibility Standards.
We are expecting the new website to go live by the end of this week. The website address will remain the same (www.cerneabbassurgery.co.uk), so no need to update your bookmarks!
You can see a preview of the new homepage in the picture below – there might be some slight changes between now and when we go live, as well as once it is available to everyone, but the format will remain the same.
Don’t be alarmed if you see this new site – it is really us!
